Direct Support Professional - Assisted Living Services
II. Definition and Basic Function: A Direct Support Professional who assists in the supervision, care and activities of the Individuals in their home or community according to their specific needs
III. Accountability: Direct accountability to the Head of Residential Services and ultimately the Director of Operations.
IV. Responsibilities:
A. Demonstrates a respectful positive attitude that promotes service excellence to Individuals, fellow Team Members, families and/or the general public
B. Follow through on responsibilities assigned by the Head of Residential Services.
C. Participate in Individuals’ program planning.
D. Direct communication of important information to the Head of Residential Services Manager by way of monthly supervision meetings and checking work email weekly.
E. Supervise Individuals with medication administration and support if needed or requested.
F. Assist Individuals with activities of daily living, if needed or requested including, but not limited to:
1. Assisting with food preparation and meal planning, shopping.
2. Assisting with personal care needs
3. Assisting with household tasks.
G. Assist Individuals with financial tasks as needed or requested including, but not limited to
1. Assisting in organization of financial records
2. Demonstrating or assisting in the use of money in an exchange of money for goods/services in the community.
3. Educating the Individual in using money
H. Supervise Individuals in the community to ensure health and safety.
I. Record activities of Individuals accurately in a daily service note recorded at the end of each shift with an Individual.
J. Complete progress notes of Individual’s outcomes on a monthly basis.
K. Maintain documentation for behavioral support plans, quarterly reports, and other necessary documentation for Individuals.
L. Attend team meetings regularly and Individual team meetings as needed.
M. Handles emergencies as they arise and follow through on proper reporting and documentation.
N. Understand and follow Friendship Community’s Privacy Policy Statement based on HIPAA regulations.
O. Understand and follow Supported Living, Residential Services Policy and Procedures in addition to Friendship Community’s Policy and Procedures.
P. Other duties as assigned by the Head of Residential Services Manager.
Q. Learn and understand the services and definitions of services that Supported Living requires.
V. Work Schedule: Work hours are determined by the needs of Individuals per number of homes scheduled as outlined in the ISP. Hours can range from morning through evening. Hours may change due to Individual needs. In the spirit of teamwork, it is expected that Team Members work on short notice in the event of an emergency with an Individual. Some Individuals may also receive supports through Technology First Principle. Enabling Technology may only be used if it is incorporated into the ISP.
VI. Education/Experience Requirements: The Support Professional must have a high school diploma/ GED, be at least 18 years if age and posses a valid driver’s license. He/she must have a vehicle in good repair and valid car insurance. The mileage for the use of the vehicle will be paid to the employee on a quarterly basis. Previous experience with persons with developmental disabilities is preferred. The SP must show the ability to be responsible, trustworthy, positive, industrious and must be able to work as a team with persons in administrative positions as well as other SP’s.
