I) TITLE: Incident and Quality Compliance Advisor

II) DEFINITION: This position supports the organization’s work in Compliance Incident and
Quality management.

III) POSITION SUMMARY: The Incident and Quality Compliance (IQM) Advisor is a
dual-track role responsible for safeguarding the health and safety of individuals while ensuring
the organization remains in total alignment with state regulatory standards. This position
centralizes the lifecycle of incident reporting with systemic quality improvement, serving as
the primary developer of the organization’s Quality Management Plan and the lead liaison for
state audits and reviews.
The IQM must be a highly composed professional capable of managing competing priorities
within a fast-paced but flexible schedule. A primary requirement is the authentic endorsement
and active promotion of Friendship Community’s mission and vision. The ideal candidate will
possess a strategic mindset, translating the organization’s long-term goals into collaborative
action and driving a culture of continuous excellence across all departments.

IV) REPORTS TO: Director of Operations

V) RESPONSIBILITIES:

a) Incident Management & Oversight
a. Investigative Supervision: Provide direct oversight and guidance to two to four
Certified Investigators (CIs). Ensure all internal investigations are thorough,
objective, and completed within the 30-day regulatory window.
b. Reporting & Lifecycle: Manage the end-to-end lifecycle of reportable events in
the Enterprise Incident Management (EIM) or HCSIS systems, ensuring all initial
and final reports meet strict regulatory timeframes.
c. Triage & Safety: Immediately assess incidents upon discovery to verify that
emergency procedures were executed and that the immediate health and safety
needs of the individual were met.
d. Corrective Action: Develop and monitor the implementation of corrective action
plans (CAPs), ensuring root causes are addressed to prevent recurrence.
e. Regulatory Liaison: Act as the primary point of contact for Administrative Staff
(AS) and the Office of Developmental Programs (ODP) regarding specific
incident inquiries.

b) Quality Assurance & Systems Improvement
a. QA&I Leadership: Lead the organization through the annual Quality
Assessment and Improvement (QA&I) process, including self-assessments and
desk reviews.
b. Internal Auditing: Conduct routine record reviews and on-site inspections of
service locations to ensure compliance with waiver requirements and 6100/6400
regulations.
c. Trend Analysis: Aggregate and analyze incident data and performance metrics
to identify systemic trends; use these findings to draft and update Friendship
Community’s Quality Management Plan and present to the Director of
Operations.
d. Staff Training: Develop and facilitate training modules for staff on incident
recognition, reporting protocols (per the IM Bulletin), and annual compliance
requirements.

VI) STATUS: Full-time, exempt located at the Oregon Pike Main Office. The hours are between
8:00 am and 4:30 pm, Monday- Friday. Occasional hours outside of normal schedule may
apply. Routine presence in the homes is required.

VII) CERTIFICATION/EDUCATION/EXPERIENCE REQUIREMENTS:
a. Education: Bachelor's degree in human services, Psychology, Social Work, or related
field.
b. Experience: A minimum of 3+ years of professional experience in the Intellectual
Disability/Autism (ID/A) field.
c. Regulatory Knowledge: Deep expertise in ODP Bulletins (specifically 00-21-02) and
Chapter 6100, 6400, or 2380 regulations.
d. Technical Proficiency: Demonstrated experience navigating EIM/HCSIS and Pelican
and managing complex data sets for reporting.
e. ODP Quality Management Certification
i. Candidates must either hold this certification or be able to obtain it within 6
months of hire. This requires:
1. Completion of prerequisite modules 101, 102, and 103.
2. Attendance of a 2-day virtual state-led class.
3. Passing the certification exam with a score of 84% or higher.
f. Certified Investigator Certification within 6 months of hire.
1. Attendance of a 3-4 full day session
2. You must achieve at least a score of 80% or higher.

VIII) SKILLS:
a) Executive Communication: Demonstrate exceptional written and verbal communication
skills, with the ability to articulate complex regulatory concepts to diverse audiences.
b) Project Leadership: Proven ability to orchestrate multiple high-stakes projects
simultaneously while meeting rigorous state-mandated deadlines.
c) Team Development: Skilled in cultivating and leading high-performing teams to achieve
strategic organizational objectives.
d) Cross-Functional Collaboration: Ability to build rapport across all levels of the
organization to design and implement systems that drive excellence in service provision.
e) Technical Proficiency: Advanced command of the Microsoft Office Suite (Word, Excel,
Outlook, PowerPoint) for data analysis and professional reporting.

as necessary for on-site inspections or record reviews.
c. Lifting & Strength:
i. Must be able to occasionally lift and/or move objects weighing up to 25 pounds.
d. Work Environment:
i. The typical work environment maintains a moderate noise level, consistent with a
professional office or community-based setting.
