Home and Community Professional
The home and community professional at Friendship Community will support the individuals enrolled in day services including the Friendship Heart Studio (FHS) and Home and Community Services (HCS) in building connections with their local community by participating in activities, building relationships and social networks, and engaging in adult learning opportunities. The home and community dupports shall be flexible, responsible, trustworthy, positive, and diligent. They shall also demonstrate initiative, candor, emotional intelligence, and have the ability to relate professionally and build relationships with those around them.
ACCOUNTABILITY:
The home and community professional is accountable to the home and community supports manager and the day services coordinator.
AUTHORITY:
The home and community professional assists in providing quality care and supports to individuals attending FHS and HCS. The home and community supports professional works in cooperation with fellow HCS team members and the art instructors, program specialists and studio managers.
QUALIFICATIONS:
The home and community supports professional shall be at least 18 years of age, possess a high school diploma or equivalent, and have a valid Pennsylvania driver’s license. Additional education and experience in human services preferred. The home and community supports professional shall be responsible, trustworthy, positive, and diligent, demonstrate initiative, and have the ability to relate professionally to the Individuals and other team members. The home and community supports professional must also be a role model in all aspects of care, support the mission, vision, and values of Friendship Community, and relate to family members, as well as other professionals involved in the individual’s care, from a customer service prospective.
RESPONSIBILITY:
A. Demonstrates a positive and respectful attitude that promotes service excellence toward individuals, team members, families and/or the general public
B. Works with the home and community supports manager to explore and implement new and positive volunteer or community opportunities for the individuals
C. Create and facilitate integrative community experiences for Individuals; encourage social interaction with community members, help individuals develop independence and new artistic and/or job-related skills.
D. Transports individuals to and from off-site outing using Friendship Community vehicles
E. Documents daily tasks and activities in a daily service notes (DSN) format
F. Documents progress notes for all Individual’s service participation (CPS and IHCS)
G. Reports medical observations and concerns to studio managers, home and community supports manager, and/or other designee
H. Assists with general upkeep of the program and other duties as assigned.
WORK SCHEDULE:
The home and community support professional is a part-time, hourly position. Flexibility may be needed for additional hours contingent upon the needs of the program and the community interest of the individuals. The home and community supports professional will be asked to work special events as needed.
